The Respondent Form
The respondent form is where you answer assessment questions and provide evidence. Understanding its layout helps you work efficiently.
Form Layout
The form is organized to match the template structure:
- •Left sidebar — Shows all domains with progress indicators
- •Main area — Displays questions for the selected domain
- •Top bar — Shows assessment name, due date, and overall progress
Navigating the Form
- •Click a domain in the sidebar to jump to that section
- •Use Next and Previous buttons to move between domains
- •Progress is automatically saved as you work
- •A progress bar shows your completion percentage
Question Display
Each question shows:
- •The question text and any guidance
- •The expected response type (text field, radio buttons, checkboxes, etc.)
- •Whether evidence is required
- •Whether it's marked as a critical gate
- •Your current answer (if previously saved)
Auto-Save
Your responses are automatically saved as you type. You don't need to manually save — just answer questions and move on. A small indicator shows when your latest changes have been saved.
Progress Tracking
The form tracks your progress:
- •Each domain shows a completion percentage
- •Answered questions are marked with a checkmark
- •Required questions that are still empty are highlighted
- •The overall progress bar updates in real-time
